I checked SC myself and pulled a couple of interviews. Yes, 36 year old actress Tânia Toko really did do everything for real with her 13 year old costar. Including the oral sex. None of it was fake. Apparently there was some shadowing digitally added to that last part after the theatrical release to make it harder to see.
I will point out that Brazilian cinema has often portrayed both boys and girls of very young age in some pretty graphic scenes. Love Strange Love (1982), A Menina do Lado (1987), and Fulaninha (1986) are three examples I can name off the top of my head. Brazilian attitudes toward sex are closer to Polynesian culture than to European or certainly American. Still, they're stricter with minor actors now than back in the 80's when those other movies were made. This one was filmed in 2005. It's the parents of the teenage boy involved being okay with it I find interesting. I assume the boy himself probably had no objections. I wouldn't have, at his age.
