What exactly does "verified" mean in this context?

Verified means, that's the file has a correct file name, but the content may be fake (a different movie or even porn). It happened a lot in the past, but that hardly ever happens nowadays.

Who verifies files, and how?

We admins and moderators. If the file is in a REQ forum we change the header to "REL" and movie the thread to a REL forum.
